You can download and print the registration form below. To register, you must print recto-verso one copy per person, fill it in, and sign it. For children under the age of 14, mom or dad must sign, stating mom’s or dad’s name and national registration number. Make an appointment for yourself and bring the identity card and signed registration form for each family member you wish to register.
